What the Dementia Service will provide
- Skilled person-centred assessment of the needs of family carers and people with dementia whose needs are complex
- Promoting the physical, social and psychological health and wellbeing of carers and people with dementia
- Assisting carers and families in their knowledge and understanding of dementia, helping them to develop their range of skills and coping strategies
- Working with carers and families in identifying their own needs, and balancing those needs with the needs of the person they care for
- Enabling carers and families to access appropriate and timely information about dementia and the range of services available to help them
- Facilitating access to professional care and resources available to the carer in the appropriate amount of detail and in a way that can be easily understood
- Supporting carers and families through the complexity surrounding loss, transitions and changing relationships
The Admiral Nurse also aims to optimise the outcomes for families affected by dementia by leading, influencing and consulting on the delivery of person-centred and relationship-centred care working alongside other health and social care professionals.
Direct work with families
The Admiral Nurse will work directly with families through a mixture of home visits or appropriate appointments in an agreed facility. Appointments will be made available within the main office and sometimes more remotely. The Dementia Advisors and Admiral Nurses will also offer telephone calls and video consultations for people who are unable to come to appointments or have home visits, due to their personal health needs or local restrictions.
